CONSENT

pursuant to Article 6(1)(a) and Article 7 of Regulation (EU) No 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) and pursuant to Article 13(1)(a) and Article 14 of Act No 18/2018 Coll. on the Protection of Personal Data and on Amendments and Additions to Certain Acts

As the data subject, I hereby give my voluntary andexplicit consent to the processing of my personal data within the scope of the e-mail addressyou have provided.

To the controller Ministry of Health of the Slovak Republic, Limbová 2, P.O.BOX 52, 837 52 Bratislava, ID No.: 00165549, tel.: 02/593 73111, telephone 02/593 73 11, e-mail: office@health.gov.sk (hereinafter referred to as the "controller") for the purpose of logging in and managing your account on the portal within the website www.cksdrg.sk for the period until the revocation of the deposited consent.

You may withdraw your explicit consent at any time by sending a request to the Operator's e-mail address office@health.gov.sk or to the Operator's registered office at the Ministry of Health of the Slovak Republic, Limbová 2, P.O.BOX 52, 837 52 Bratislava.

As the data subject, I declare that I am aware of my rights under Act No. 18/2018 Coll. on the protection of personal data and on amendment and supplementation of certain acts.

The controller shall ensure that the personal data of the data subject are sufficiently protected against theft, against accidental or intentional access by an unauthorised person, as well as against any other inadmissible forms of processing. It shall create conditions such that the confidentiality and integrity of personal data are not compromised in the processing of personal data.

   7.4 the right to object to the processing of personal data (§ 27 of the Act),

   7.5 the right to the portability of personal data (§ 26 of the Act),

   7.6 the right to withdraw consent at any time (§ 14(3)),

   The controller does not process the personal data of the data subject by automated individual decision-making, including profiling.
